INSURANCE SUMMARY

There is a 90-day waiting period for the following
Medical Benefits
  • Preferred Provider Organization, Managed Care, or HMO
  • Individual, Individual Plus Dependent and Family
  • Prescription Drugs Program
Dental Benefits
  • Basic, Comprehensive or DMO
  • Individual, Individual Plus Dependent and Family
  • 80% coverage of basic services
Life Insurance
  • 100% of salary up to $50,000.00
Short-Term Disability (self-funded)
  • From 6th to 90th consecutive workdays paid as percentage of weekly earnings (determined by length of service) up to a maximum of $500.00
Long-Term Disability
  • From 90th workday; 50% of base pay up to $10,000/mo.
  • Additional LTD Benefits available

Credits are provided to each full-time employee to purchase standard health/hospitalization coverage as described in the “Insurance Summary” section.

COMPENSATION

There is a 90-day waiting period for the following:
401(K) Plan
  • Employee can contribute up to 10% total compensation (fully vested)
  • will match employee contribution 1.00:1.00 up to 5% of employee’s total salary (vesting schedule for years of service)
Incentive Bonus Plan
(outstanding performance, business development)

Policies of Leave

Paid Time Off
  • 15 Days/Year
  • Used for vacation, illness, and personal appointments
Holidays
